Here’s the official synopsis for the episode, per Prime Video:

It’s Election Night and President-Elect Bob Singer and VP Victoria Neuman are throwing a party to celebrate their victory! But when The Boys crash the party to assassinate Neuman, the mission goes horribly wrong. Now the clock is ticking to finish the job before the election is certified on January 6th, when Neuman will be one head-pop away from the Presidency. Meanwhile, Butcher’s health has badly deteriorated from his Temp V use in Season 3, and he’s running out of time in his quest to save Ryan, who’s fallen further under Homelander’s malign influence.

Annie, having officially left The Seven, is trying to distance herself from the role of “Starlight” and just be Annie January — but leaving the supersuit behind is proving harder than she thought. After a family crisis, Hughie gets a visit from the last person he ever expected to see again. And when Homelander finds a gray hair, he’s confronted with his own mortality, and worries about the world he’s going to leave behind for Ryan. So he recruits Sage, a superhero and the world’s smartest person, and with her help, Homelander becomes more dangerous than ever.

The world is on the brink. Victoria Neuman is closer than ever to the Oval Office and under the muscly thumb of Homelander, who is consolidating his power. Butcher, with only months to live, has lost Becca’s son as well as his job as The Boys’ leader. The rest of the team are fed up with his lies. With the stakes higher than ever, they have to find a way to work together and save the world before it’s too late.
